OK, you leave the nut off a leading edge flap roller. When the flap retracts the bolt falls into the track rail and is pushed back and punctures the fuel tank. In flight no problem, but when you park the fuel now runs onto the hot tail cone. 3.5 minute response for fire Dept. What a sad fate for a Boeing 737-800.
